PATIENTS

YOUR MEDICAL RECORD

The medical file is a file in which the different professionals, in particular doctors, will gather all the documents concerning you. It is a personal document, to which you can have access.

  • the medical document indicating the reason(s) for previous consultations
  • the medical document indicating the reason(s) for hospitalisation;
  • the hospitalization report specifying the discharge diagnosis and including in particular :
  • the conclusions of the main clinical examinations carried out;
  • reports of para-clinical explorations and significant complementary examinations;
  • indications and precautions for follow-up ;
  • the operative or delivery report(s);
  • documents relating to anaesthesia ;
  • therapeutic prescriptions and orders, including discharge orders;
  • documents mentioning transfusion procedures;
  • the nursing liaison sheet ;
  • as appropriate, other significant documents (imaging films, etc.).

The medical file is a personal document. The doctor who takes care of you at the Clinic is the first person to have access to it, as he or she fills it out. At the end of your hospitalisation or consultation, a discharge prescription may be given to you, together with information useful for the continuity of your care. A hospital report must be sent to the doctor you have designated. You can ask for a copy. A copy of the elements of your medical file can also, at your request, be sent to your doctor.

Apart from the doctor you have appointed, you are the main person who can access your medical file.

The documents in the file are protected by confidentiality rules and may not be communicated to third parties. They may also contain personal information about other people (e.g. your family) which will not be passed on to you.

If the patient did not object during his or her lifetime, the heirs of a deceased person can only be given certain documents concerning him or her. Only those documents that enable the reasons for the death to be known, the patient's memory to be defended or a right to be asserted will be transmitted. The rightful claimant must also specify the objective he or she is pursuing.

Your medical file cannot be communicated to another person without your agreement.

You can request access to your medical records.
  • directly at the clinic: you can consult it on site. Make an appointment with the relevant medical department, either directly or through the clinic's management. If you wish, a doctor can be present to answer your questions. Copies will be given to you at your request.
  • by post, on written request, accompanied by a copy of your identity card. The request must be addressed to the Clinic, specifying the medical department concerned. You can also mandate a person to obtain a copy of your medical file, provided that he/she can prove his/her identity and express mandate.
For your information, copying and mailing costs are chargeable.

PROTECTION OF YOUR PERSONAL DATA

The PROCREA Clinic is concerned about the protection of personal data. It
undertakes to ensure the best level of protection of your personal data in
compliance with the Data Protection Act.
In particular, we collect and process administrative, social and medical data.
The data is used to facilitate the administrative management of your file, the billing of medical acts, the teletransmission of care sheets, the publication of test results, statistical work for the analysis of the activity of the services.
To ensure effective protection of privacy and individual freedoms in the ICT environment in Côte d'Ivoire, the ARTCI (AUTORITÉ DE RÉGULATION DES TÉLÉCOMMUNICATIONS/TIC DE CÔTE D'IVOIRE), in application of law n°2013- 450 of 19 June 2013 on the protection of personal data, is responsible for verifying the legality of the collection and use of all personal data and for ensuring that all precautions are taken to prevent the data from being distorted, stolen, sold or communicated to unauthorised persons.
 
You have the right to access, rectify and object on legitimate grounds to your personal data.
You can exercise this right by contacting the Director of the Procréa Clinic where you were treated.
All departments at Clinique Procréa have an IT system for managing patient files.
 
This system manages all the data necessary for patient care. It also enables administrative management, invoicing of procedures and research or analysis of care.
The administrative, social and medical information that we collect from you and that concerns you (or your child) is regularly recorded in this system and is processed under the responsibility of the Procréa Clinic.
 
Only the health professionals of the care team involved in your care, or that of your child, can access the medical data. In order to improve the quality of your care, the Clinic may also, with your explicit agreement, transmit this data to health professionals outside the Procréa Clinic who are also involved in your care. In this context, information concerning you may be deposited with a data host approved for this purpose and processed by partner organisations.
 
The medical file is kept, in accordance with the Public Health Code, for a period of twenty years from the date of the last visit, or at least until the patient's twenty-eighth birthday, or for ten years from the date of death.

Everyone is free to choose the health care facility that will take care of them

The PROCREA clinic guarantees the quality of its reception, treatment and care.

1. All persons are free to choose the health care facility that will care for them.
2. PROCREA clinic guarantees the quality of the reception, the treatments and the care.
3. The information given to the patient must be accessible and fair.
4. A medical act is only performed with the free and informed consent of the patient.
5. Specific consent is required for certain procedures, in particular for patients making a donation or for the use of elements or products of the human body.
6. Biomedical research cannot be carried out without the person's consent after having been specifically informed of the expected benefits, constraints and foreseeable risks.
7. The hospitalized person may, at any time, leave the PROCREA clinic if he/she wishes, provided that he/she has been informed of the risks involved and has signed a discharge releasing the clinic from responsibility.
8. The hospitalized person is treated with respect regardless of his/her political, religious or sexual orientation.
9. PROCREA clinic guarantees the respect of privacy to all its patients.
10. The hospitalized person (or his/her legal representatives) has direct access to health information concerning him/her through his/her attending physician.
11. The hospitalized person expresses his observations on the care and reception and is required to comply with the internal rules of the PROCREA clinic.

DESIGNATE ATRUSTEDPERSON

During your stay, to accompany you throughout your care and the decisions to be taken, you can designate a "trusted person".

This person can be freely chosen from among your family and friends and, if you so request, he or she can attend medical appointments and help you to formulate your wishes. Your state of health may not always allow you, during your hospitalization, to make your opinion known to the people who care for you or the decisions you wish to make regarding your care.

The appointment of a trusted support person is not an obligation and should be a well-considered decision, without being rushed. If you wish to appoint a trusted support person, discuss this with your doctor.

Gynecologist-obstetrician

Dr JLADIKPO is an obstetrician-gynecologist with over 10 years' practical experience. She holds a Diplôme d'Etudes Spécialisées in Gynecology-Obstetrics, a Diplôme Universitaire de l'Infertilité et Aide Médicale à la Procréation from the Université Paris Descartes and a Diplôme Universitaire de Colposcopie et Pathologie Cervico-vaginale from the Université d'Angers. 

She has several training courses to her credit, notably in Sexual and Reproductive Health Rights and in Health Human Resources Management. She will join PROCREA's dynamic team on May 02, 2023.

DR. GLIDJA RODRIGUE ANGES KOSSI

PEDIATRIC

Dr. Glidja is a pediatrician with 10 years of experience in the field of general pediatrics. With international experience in the sub-region, he works with patients from diverse backgrounds and cultures. Along with from his DES in Paediatrics at the Faculty of Health Sciences of the University of Lomé, he has several training courses under his belt, notably in the field of Tuberculosis and Communication Techniques.

He will join the dynamic team of the PROCREA Clinic on January 10, 2022.

Medical Director

Doctorate in Medicine, Faculty of Medicine, Abidjan. He previously held the positions of Advisor to the Deputy Technical and Commercial Director, Head of the Health and Social Protection Department at SIDAM, and Resident Doctor at La Madone Clinic from 1988 to 1998.

Clément Oussou is an associate doctor and one of the consultant specialists in obstetrics and gynaecology and is also a specialist in video surgery.

GENERAL SURGEON/Head of the General Medicine Department

Dr. Amos-Djoro is a specialist in general medicine with over 35 years of experience in the field. practice in the field. He graduated from the Faculty of Medicine in Tours, France.He practiced medicine in a multidisciplinary health center in the Cher in the Cher department and then in a medical centre in the Nièvre department, between 1996 and 2021. With this long experience as a general practitioner, he joins PROCRÉA clinic on October 1st 2022 as Head of the General Medicine Department.

Dr. NASSIROU ADEBAYO FAISSAL

PEDIATRIST - Head of Paediatrics

Dr Nassirou is a paediatrician with more than 15 years of experience in the field of neonatology and paediatrics in general. In addition to the DES in Medical Paediatrics and Childcare, he has several diplomas to his credit, including a DES in Bacteriology and Virology from the University of Félix Houphouët in Abidjan, a DIU (post-graduate diploma) in Perinatal Endocrinology from the University of Paris Saclay and a training course in Resuscitation of the newborn from the University of Aix-Marseille -Provence Medical Relations.

He joined PROCREA Clinic on November 1st 2021 as Head of the Paediatric Department.

Dr MAMY holds a specialized diploma in obstetrics and gynaecology. With 9 years of professional experience, he joined the PROCREA Clinic team in December 2020.

Physician / Intensive care anesthesia

Dr JOEL KOUASSI holds a doctorate in medicine, a state diploma, a certificate of specialized study in anesthesia resuscitation and a university diploma in disaster medicine.

He joined the procréa clinic on January 02, 2023 as a resident anesthetist with 28 years' experience in anesthesia and intensive care.

Dr. Mariame SAKO

Gynecologist - General practitioner

Dr. Mariame SAKO holds a doctorate in medicine from the Free Faculty of Medicine of Lille. During her studies she specialized in emergency and disaster medicine and medical gynecology among others, with an experience of more than 10 years in the mentioned fields.

Dr. Marie JEANSON

General Practitioner - Aesthetic Physician

Dr. Marie JEANSON holds a diploma of specialized study in

General Medicine from the Université Paris Est Créteil. During her studies, she specialized in aesthetic and reparative facial injections at Paris Descartes University.

She joins the PROCRÉA clinic on March1, 2023 as a resident general practitioner on the inpatient ward.

Dr. Ahoui KONAN

Chief Pharmacist

Dr. Ahoui KONAN holds a doctorate degree in pharmacy from the University Félix Houphouët Boigny of Abidjan in 2008. He worked as a pharmacist assistant from 2008 to 2010, then as a civil servant from 2010 to 2014, and as a pharmacist from 2014 to 2021. On March 1, 2023 he joined PROCREA clinic as Chief Pharmacist.

Mrs. KARIDIATA KONE

Midwife

Mrs. KARIDIATA KONE is a midwife with a state diploma in INFAS since 1990, trained by the World Bank on the quality of services delivered in health facilities; she is a referent trainer of midwives and quality supervisor for the district of Cocody-Bingerville. She has 33 years of experience.

Mr. Oumar KONATE

Midwife

Mr. Oumar KONATE is a state-qualified midwife from the ENPS (National School of Public Health) of Bobo Dioulasso since 2005 with several experiences as head of gyneco-obstetric services, hospitalizations and assistant supervisor of care units; he has 17 years of experience.

DR. Mantary SOUMAHORO

Pharmacist Biologist - Qualitician

Dr. Mantary SOUMAHORO is a pharmacist-biologist and quality control specialist.

She holds a PhD in Pharmacy and a DU in Quality Assurance of Medicines from the Faculty of Medicine and Pharmacy of Rabat, with a ten-year seniority.

During her studies, she specialized in Clinical Biology: 

DES in Clinical Biology from the University Félix Houphouët of Abidjan, and in AMP: DU in Infertility Gynecology and AMP from the University Paris Cité.

She will join PROCREA on SEPTEMBER 1, 2022 as a Pharmacist-Biologist and Deputy Head of MPA activities.

Specialist in psychiatry since 2004

She has been involved in psychological support for couples in the area of fertility since 2016.

DR. JOSE AMOS DJORO

Dr. Myriam KADIO-MOROKRO

Reproductive biologist

General Manager of the PROCREA Clinic, Vice-president of the LYA Foundation - living with sickle cell disease

Holder of a Doctorate of Medicine from the Faculty of Medicine of Abidjan since 2000, Myriam Kadio-Morokro then enrolled in the Training and Research Unit of Medicine Pierre and Marie Curie - Paris XIII from where she obtained the diploma Medicine and Biology of Reproduction in 2003.

With this diploma and after further training in Paris, she returned to Côte d'Ivoire in 2005. She is first head of the classical biological analysis laboratory of the BLEUETS medical center in Abidjan.

Then in 2008, she created with 2 other partners the Ivorian company of hospital management (SIGH), a limited company with a board of directors which is in charge of the PROCREA clinic, the first fertility center with Ivorian capital;

For the past 10 years, she has been the General Manager and responsible for the general strategy of the clinic. She also works as a reproductive biologist.

With her expertise in reproductive health, she participated in 2009 in the creation of GIERAF (Inter African Group for Study, Research and Application on Fertility) with 8 African countries. She was the General Secretary for 4 years.

PROCREA clinic has obtained the GIERAF label since April 2019. Indeed, GIERAF has validated during an audit, the quality approach of the processes in Medical Assistance to Procreation, in accordance with international standards required making the clinic the1st fertility center to have been labeled in Côte d'Ivoire.

In 2019, she obtained the Executive Must certificate on the management of a strategic unit delivered by HEC Paris.

Leader in its field in Côte d'Ivoire. It organizes in October 2022, the inauguration of its new clinic; PROCREA 2. Meeting international norms and standards, it will be better equipped and more capable of receiving all men and women from Africa and elsewhere in search of a parental project. This field of activity requires know-how, confidence and experience.

In parallel to these medical activities, and touched by the disease of one of her relatives and with a purely humanitarian aim, Dr. Kadio-Morokro decided with a group of people to participate in the creation of the LYA FOUNDATION - living with sickle cell disease, which contributes through its action to a better management of sickle cell disease

She has been its President since 2021. She is involved in field actions and projects on sickle cell disease.

Its experience is associated with the mobilization of concerned persons, public authorities, health professionals and social-health actors.
